We have extensive experience of supporting those who have been in hospital as well as those receiving mental wellbeing counselling by offering a range of services including:
Carrying out home safety checks to identify safety hazards, including trips and fall risks, health risks, lack of necessary facilities and evidence of fuel poverty.
Sourcing funding for necessary cleaning and clearance, repairs and adaptations.
Undertaking repairs, maintenance and adaptations prior to the patient returning home.
Conducting one-to-one case work from our trusted assessor-trained care managers, including information and advice relating specifically to independent living.
Signposting and referral of patients to other statutory, voluntary and charitable agencies for additional ongoing support and assessment.
Our services are designed to support NHS Trusts, community and private hospitals as well as mental health practices. Our track record in the healthcare sector includes Bristol Royal Infirmary, Southmead Hospital, South Bristol Community Hospital, Royal United Hospitals, St Martins Hospital, Paulton Community Hospital and the Royal National Hospital for Rheumatic Diseases.
We can take referrals at any point in the patient journey but the sooner the better.
